If your name is on the list below, please contact the Town Accountant’s Office at 978- 694-2029 for further information on filing a claim for a replacement check. You can download a Claim Form (PDF) at the Town’s website (www.wilmingtonma.gov) or request a form by calling the Town Accountant’s Office at 978-694-2029.
Claim Forms must be submitted with all the information completed and an original signature. Electronic copies, photocopies and faxed copies will not be accepted.
