March 16, 17, 18, 19, 20, 2015

TIMES:

Boutwell Early Childhood Center — 8:30 AM to 10:30 AM, 12:30 PM to 2:00 PM

Wildwood Early Childhood Center: 9:00 AM to 11:00 AM, 12:45 PM to 2:15 PM

AGE  REQUIREMENT: 

Children must be 3 years old by August 31, 2015

TUITION:

Current Tuition is $2,730.00. Tuition for 2015-2016 to be determined.

AM SESSION                    8:00 AM  to  10:30 AM             8:30 AM  to 11:00 AM

PM SESSION                  11:05 AM  to    1:35 PM           11:30 AM   to  2:00 PM

WHAT IS NEEDED TO REGISTER YOUR CHILD FOR THE PRESCHOOL LOTTERY

  • Original birth certificate with official seal
  • Proof of Occupancy (one of the following items)
  1. Gas/Oil Bill, Electric Bill, Home (not cell) Telephone Bill, Cable Bill, Water Bill (Note: Bill must be dated within the past 45 days and address and name must be stated)
  2. Recent bill (other than that stated above dated within the past 45 days showing Wilmington address and name. However, a Residency Statement/Affidavit is REQUIRED with this option. Please refer to district’s website and click on RESIDENCY POLICY & PROCEDURES to print form to be used.)
  3. Occupancy Statement/Affidavit must be notarized if a bill can not be provided prior to student’s enrollment (please refer to district’s website and click on RESIDENCY POLICY & PROCEDURES to print form to be used “Occupancy Statement/Affidavit”)
  • $25.00 Registration Fee (check or money order, No Cash Please) payable to: Wilmington Preschool

IF YOUR CHILD’S NAME IS DRAWN IN THE LOTTERY THE FOLLOWING PROOFS OF RESIDENCY MUST BE PROVIDED WITHIN TWO WEEKS OF THE DRAWING

  • Proof of Residency. (one of the following items)
  1. Record of recent mortgage payment and/or property tax bill
  2. Fully signed and executed Lease and/or Rental Agreement (Must be executed by both parties)
  3. Notarized Landlord/Owner of Property Affidavit (please refer to district’s website and click on RESIDENCY POLICY & PROCEDURES to print form to be used “Residency Statement/Affidavit”)
  4. Fully signed and executed Purchase and Sale (P&S) Agreement (provided occupancy date occurs within 30 days of enrollment)
  5. Section 8 Agreement
  • Proof of Identity (one of the following items)
  1. Valid MA Driver’s License
  2. Valid MA Photo ID Card
  3. Valid Passport
  4. Other Government issued Photo ID
